Hours: 22.5 hours per week (0.6 FTE)
Contract: Fixed term until September 2025 (with potential to extend subject to funding)

Project Summary:
High intensity is the effort we put into people. By concentrating on people and what they want, results follow.
The overall aim of this project is to work alongside those people who are the highest users of the emergency department in Exeter to enable them to address some of the issues that have led to that use. We aim to break the cycle of repeated presentation and help individuals to achieve a resolution which is meaningful to them. The successful applicant will listen to the individual and bring together key practitioners to ensure they are all working collaboratively for the interest of the individual. The project is a partnership between Encompass Southwest and CoLab and will be hosted by CoLab through the innovative One Mental Health Team project.

About the role:
We are seeking to appoint an individual who can demonstrate a person-centred approach towards individuals who we serve, many of whom who have complex and challenging lives. This is an exciting opportunity to be part of a team aimed at reducing inequalities and promoting inclusion to some of the highest need clients within our community. More than that, High Intensity User work is proving to be some of the most impactful around the country, and this role brings the opportunity to see this service embed, develop, and flourish within Exeter.
The successful applicant will work in partnership with CoLab and the services based there, as well as other public and VCSE service partners, to deliver a holistic social prescribing service to clients, understanding the person, individual circumstances and life experiences supporting them to meet their goals enabling them to live the life they want.
