One Mental Health Team

Project Information

One of the shared aspirations we have long had as a community has been to improve the response to mental health challenges for our most vulnerable people. This one year year pilot (funded by DPT and the National Lottery) aims to coproduce and test an aligned offer, which draws the best from available skills and expertise, and gathers evidence of what works for people and for joint working. this will inform future decisions about funding and provisions.

The initial service offered is for people who are homeless, vulnerably housed and experiencing complex needs. People who may require support or signposting alongside community based opportunities which aid recovery and build resilience.

The team is hosted at CoLab and supported by the CoLab core team. Team members include: -

  • Specialist Primary Care Support - The Clocktower Surgery

  • Specialist Mental Health Support - The DPT Homeless Mental Health Team

  • Specialist Community Mental Health Support - Devon Mind

  • General Community Mental Health Support and Signposting - CoLab Mental Health

Who is it for?

The Process

To access the service, partners can complete a 'Raise a concern' form (which can be found in the Links & Documents box below) including obtaining consent to share information.

People presenting at the CoLab will be welcomed by the core team and either sign-posted to services, internally or externally, or referred into the weekly allocations meeting.

The team will work collaboratively and compassionately with available clinical, experienced social and support networks and community teams to enable people to access the right support, advice and guidance-at the right time-in the right place.
